The Red Cliff Community Health Center offers a variety of outreach services for our community that have been specialized into Maternal and Child Health (MCH) and Adult Health services.
Maternal Child Health Services:
Honoring Our Children Project (HOC)
The HOC Project is a Tribal Healthy Start Project that is focused on health and social services aimed at reducing infant mortality and morbidity rates in the Native American community. The program accomplishes these goals by meeting with families during the pre-natal and post-partum periods and into the first several years of life. Services offered include; prenatal care coordination, incentives, breastfeeding education and support, childbirth education classes, injury prevention-car seat education, smoking cessation counseling, transportation and home visitation.

Adult (and Pediatric) Health Services:
Community Health Nurse (CHN) and Community Health Representatives (CHR)
The Community health team works collaboratively to provide a wide variety of health related services, with an emphasis on wellness.
Some of the services provided include; case management, immunizations, injury prevention, WIC, Optical program, diabetes support groups, employee health and in-services, community education, screening clinics, nursing consultation, transportation, home visitation, and community wellness events.
